Safety Programs and Required Plans

  • Injury and Illness Prevention Program
    Outlines the department’s comprehensive safety program—including responsibilities, communication systems, hazard identification, inspections, training, and required documentation—to ensure compliance with UC Davis policy and California safety regulations.
  • Emergency Action Plan
    Provides the department’s procedures for reporting emergencies, evacuation, communication, and employee responsibilities during incidents, ensuring compliance with Cal/OSHA requirements and UC Davis safety protocols.

Job Safety and Worksite Assessments

  • Job Safety Analysis (Office)
    Summarizes typical office hazards—ergonomic strain, slips and falls, electrical risks—and the safe practices and training needed to maintain a safe office workspace.
  • Job Safety Analysis (Lab)
    Identifies common laboratory hazards and outlines required safe practices, PPE, and training for working safely with chemicals, biologicals, radiological materials, and specialized equipment.
  • Laboratory Worksite Inspection Form
    A checklist used to evaluate lab safety conditions—including general hazards, emergency equipment, chemical storage, electrical safety, and ergonomics—to ensure compliance and identify issues that require correction.

Training and Documentation

  • Safety Training Attendance Form

  • General Safety Training Checklist
    A checklist for supervisors to document required safety training for lab and office personnel, including basic safety procedures, emergency readiness, chemical hygiene, and (when applicable) animal-related training.

Hazard Reporting and Follow-Up

  • Hazard Alert Form
    A form for reporting unsafe conditions in the workplace, documenting the location, description, and recommended corrections, along with follow-up by management or the safety committee.
  • Hazard Correction Report
    Used alongside the Hazard Alert Form, this report tracks how identified hazards are corrected, documenting required actions, responsible parties, and completion dates to ensure timely resolution.
  • Employers First Report of Injury or Illness
    Report a workplace injury or illness by providing who was hurt, when and where it occurred, the body parts affected, and whether treatment was received.
